Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted on November 18, 2019. The on-site inspection was from 1:15 p.m. to 5:40 p.m. On this date, there were 12 children in the Pre-K program with two staff members present. There were 16 children enrolled in the the Pre-K program and 160 children enrolled in Covenant Club. There were 50 children present in the after-school Covenant Club program and in the care of six staff members, with one additional staff member available to assist if needed. The exceptions to compliance were noted on the violation notice. There were five children's records reviewed and seven staff members' records reviewed. The medication and medication paperwork were reviewed for three children. There were six staff members and five children interviewed. Lunch was brought from home. Afternoon snack was provided by the center and consisted of pretzels, fresh strawberries and bananas. Some items observed/reviewed included: evacuation and shelter-in-place drills, injury reports, updated injury prevention plan and emergency preparedness plan, updated annual health inspection, required posted information, documentation of updated asbestos inspections, warm water temperature, the playground, outdoor play, nap time, transitions, arrival of after-school students, table top games and activities, afternoon snack, and dismissal procedures.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on review of seven staff records and verification from staff, one new staff member had not obtained a tuberculosis (TB) screening.

Evidence:
1. The record for Staff #3 did not contain the results of a TB screening. Date of hire for Staff #3 was 08/10/2019.
2. Staff #3 confirmed that she had not obtained a TB screening.

Plan of Correction: During the inspection, the staff member obtained a TB screening and the negative results were added to her file. In the future, any new staff member will obtain a TB screening within 21 days of hire.

Standard #: 22VAC40-185-240-A
Description: Based on review of seven staff records and verification from the director, two new staff members had not received the required orientation training.

Evidence:
1. Staff #1 and Staff #2 did not receive orientation training which is required by the end of their first day of assuming ob responsibilities. Date of hire for Staff #1 and Staff #2 was 08/12/2019.
2. The director confirmed that the two new staff members had not received the required orientation training.

Plan of Correction: Orientation training will be completed with the two new staff members as soon as possible. In the future, all new staff will be trained on the required information by the end of their first day of assuming job responsibilities.

Standard #: 22VAC40-185-240-B
Description: Based on review of seven staff records and verification from the director, two new staff members had not been provided the required written orientation information.

Evidence:
1. Staff #1 and Staff #2 did not receive orientation information in writing which is required by the end of their first day of supervising children. Date of hire for Staff #1 and Staff #2 was 08/12/2019.
2. The director confirmed that the two new staff members had not received the required orientation training in writing.

Plan of Correction: Orientation training will be completed and the written information will be provided to the two new staff members as soon as possible. In the future, all new staff will be trained and given all written information by the end of their first day of supervising children.

Standard #: 22VAC40-191-60-B
Description: Based on review of seven staff records and verification from the Business Office Administrative Assistant, a completed sworn statement had not been obtained from two new staff members.

Evidence:
1. The records for Staff #1 and Staff #3 did not contain completed Sworn Statements or Affirmation which are part of the required background checks when hiring a new staff member. Date of hire for Staff #1 was 08/12/2019. Date of hire for Staff #3 was 08/10/2019.
2. The Business Office Administrative Assistant confirmed that Sworn Statements had not been obtained.

Plan of Correction: Sworn Statements will be obtained for both staff members as soon as possible. In the future, the center knows to obtain a completed Sworn Statement before a staff member is hired, even for staff members that previously had worked at the center.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of seven staff records and verification from the Business Office Administrative Assistant, a completed search of the Child Abuse and Neglect Central Registry had not been obtained from two new staff members.

Evidence:
1. The records for Staff #1 and Staff #3 did not contain results from the Child Abuse and Neglect Central Registry which are required background checks when hiring a new staff. Date of hire for Staff #1 was 08/12/2019. Date of hire for Staff #3 was 08/10/2019.
2. The Business Office Administrative Assistant confirmed that the requests for the background checks had not been completed.

Plan of Correction: Both staff will complete requests to obtain results of a Central Registry search as soon as possible. In the future, the center knows to send a request to have the search completed when new a new staff member is hired so that the results can be obtained within 30 days.
