Comments:

The licensing inspector conducted an unannounced monitoring inspection on 11/21/2019 from 9:55am to 1:50pm. The children were eating snack and preparing to go outside upon the inspectors arrival. The children were later observed eating lunch and preparing for nap. A formal observation was completed with the Toddler I and Threes Lunch / Rest classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Six children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. Ten injury reports were reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 11/20/2019 Last shelter-in-place drill: 10/30/2019 Last fire inspection: 09/20/2019 Last health inspection: 06/14/2019 Today, the following child to staff ratios were observed: Toddler I (Toddlers, Twos and Threes) ? 12:3 Toddler Lunch / Rest (Toddlers, Twos and Threes) ? 13:3 Threes Lunch / Rest ? 18:3 Fours Lunch / Rest: 10:2 The violations from the previous inspections were checked for corrections. Description:	Based on observation, the program did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: The inspector observed several cleaners located in an unlocked cabinet in the four year old Lunch / Rest room. The cabinet was within the reach of children in care.
Plan of Correction:	We'll schedule the contractor to come and do the work.
Standard #:	22VAC40-185-330-B
Description:	Based on observation, the program did not ensure that resilient surfacing complied with minimum safety standards. Evidence: 1. The inspector measured several areas of resilient surfacing on the Toddler playground. The rubber mulch on the left side of the blue stationary car measured about 3.5 inches of rubber mulch. The area surrounding the Circle Cycle Merry-Go-Round measured about 4 inches of rubber mulch. Six inches of rubber mulch is required to surround the above referenced playground equipment. 2. There were layers of wooden mulch and rubber mats under The House Playground swings. The inspector was unable to determine if the swings had adequate resilient surfacing underneath the swings and in the fall zones.
