Comments:
The licensing inspector conducted an unannounced monitoring inspection on 12/02/2019 from 11:00am to 4:30pm. The children were observed participating in story time, eating lunch and preparing for nap. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today.

Ten children?s records and ten employee records were reviewed during this inspection. Ten injury reports were reviewed. Medication is being administered and was reviewed.

The center's first aid kit and emergency supplies were inspected.

The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not ensure that two of ten children's records contained documentation of additional immunizations once every six months for children under the age of two years.
Evidence:
1. The record of child #2 (start date: 7/3/18) had documentation of immunizations dated 6/20/18 and 2/20/19. Additional immunizations should be obtained every 6 months for children under the age of two years. At the time of inspection child #2 was about 20 months old.
2. The record of child #4 (start date: 7/10/18) had documentation of immunizations dated 7/3/18. Additional immunizations should be obtained every 6 months for children under the age of two years. At the time of inspection child #4 was about 20 months old.

Plan of Correction: I will check all children's files under two and make sure to obtain immunizations every six months.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. The inspector observed an unlocked cleaner hanging on the shelf above the hand washing sink, out of the reach of children.
2. The inspector observed a cleaner located in an unlocked cabinet, out of the reach of children in care.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-185-570-A
Description: Based on observation, the center did not ensure that when a child is placed in an infant seat or high chair, the protective belt should be fastened securely.
Evidence:
The inspector observed three infant children sitting in a feeding table without their protective belt's fastened.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review, the center did not ensure that one of ten staff records contained documentation of out of state child abuse and neglect search request by the end of the 30th day of employment.
Evidence:
1. The record of staff #4 (start date: 10/14/2019) did not have documentation of an out of state child abuse and neglect search request.

Plan of Correction: We faxed the out of state request and will follow up by Friday.
