An unannounced monitoring inspection was conducted on December 4, 2019 from approximately 2:00 pm to 4:10. Three staff were present with twelve children. Five children and five staff records were reviewed.

Standard #: 22VAC40-185-70-A Description: Based on review, facility did not ensure 1 of 5 staff records has documentation of required information.
Evidence: The record for staff #2 (start date November 5, 2019) did not have documentation of orientation training.Plan of Correction: Orientation was documented and placed in record during inspection.
Standard #: 22VAC40-185-260-A Description: Based on review, facility did not ensure there is an annual fire inspection report from the appropriate fire official having jurisdiction.
Evidence: The most recent fire inspection report available for review was completed September 17, 2018.Plan of Correction: Staff will continue to call and request for an inspection to be done.
