An unannounced monitoring inspection was conducted on December 5, 2019 from approximately 9:00 am to 10:45 am. Two staff were present with twelve children. Five children records were reviewed.

Standard #: 22VAC40-185-60-A Description: Based on review, facility did not ensure 2 of 5 children's records have documentation of required information.
Evidence: The records for child #1 (start date September 5, 2019) and child #3 (start date September 3, 2019) did not have a second person to contact in an emergency if a parent cannot be reached. Standard #: 22VAC40-185-330-B Description: Based on observation, facility did not ensure where playground equipment is provided that resilient surfacing shall comply with the minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence: Measurements obtained confirmed the platform for a double slide on the large composite of play equipment is 4' high and fall zone is 7' at the end of the slide chute. Based on the height of the slide, the required fall zone would be 8' at the end of the slide chute (height of slide + 4').Plan of Correction: Not available online.
