Comments:
An unannounced monitoring inspection was conducted on 12/02/2019 from 4:45 pm - 6:00 pm. At the time of entrance there were 51 school age children in care with five staff. Children were observed playing kickball in the gym. Art, table games and home work was observed in the cafeteria.
Records were reviewed for eight children.
Three violations were observed and were reviewed with the program director at the conclusion of the inspection. These violations are listed on the violation notice.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that children's records contained all of the required elements.

Evidence:
1. The enrollment record for child 1 lacks documentation of the name, address and telephone number for a second emergency contact.
2. The enrollment record for child 2 lacks documentation of the name, address and telephone number for two emergency contacts.
3. The enrollment record for child 3 lacks documentation of the name, address and telephone number of a second emergency contact.
4. The enrollment record for child 4 lacks documentation of the name, address and telephone number for a second emergency contact.

Plan of Correction: The program director stated that each parent will be asked to provide emergency contact information within the week.

Standard #: 22VAC40-185-510-E
Description: Based on medication review, the center failed to ensure that medication shall be labeled with the child's name.

Evidence:
One container of over-the-counter medication was not labeled with the child's name.

Plan of Correction: The medication was labeled during the inspection. The medication was in a labeled plastic bag.

Standard #: 22VAC40-185-550-E
Description: Based on record review and interview, the center failed to maintain a record of the dates of the practice drills for one year.

Evidence:
1. The most recent fire drill noted on the facility emergency log was conducted on 09/20/2019.
a. There was no fire drill documented for October and November 2019.
2. The program director stated that the practice drills for October and November 2019 had not been written on the emergency log.

Plan of Correction: The fire drills for October and November 2019 will be added to the emergency log this evening.
