Comments:
An unannounced monitoring inspection was conducted on December 4, 2019 from approximately 9:45 AM to 12:20 PM to determine the center's compliance with licensing standards. The child to staff ratios were deemed in compliance with licensing standards.The following staff-to-child ratios were observed: PreK I 3:13, PreK II 2:13, 3 year olds 2:10, Toddlers I 2:12, Toddlers II 2:12. The licensing inspector observed children participating in a variety of activities. The staff were observed to be engaged with the children in age appropriate activities and to be meeting the needs of the children in care. Lunch today consisted of cheese pizza, tossed salad, oranges and milk. The facility had the following posted: license, emergency numbers, evacuation maps/procedures, daily schedule, menu, and various parent information. The first aid kit and non-medical emergency supplies were inspected and found complete. Six staff records and six children's records were reviewed. Description: Based on a review of six staff records, the licensee failed to ensure that two staff members had central registry findings within 30 days of employment. Evidence: 1. The records for Staff #1 and Staff #6 indicated a hire date of 10/2/19 and there were no central registry findings recorded.

Plan of Correction: Administration has received the Central Registry search results for both staff, however the results could not be located during the inspection. Administration requested additional copies of the results for the case records the day of the inspection.
