Comments:
An unannounced renewal inspection was conducted with the center director and center staff on December 6, 2019 to determine compliance with licensing standards. There were ten preschool children and five toddlers making holiday crafts, eating lunch, and preparing for rest. The infants had finished eating and were playing with toys. There are no medications on site today.
The last health inspection was conducted 6/3/2019. The last fire inspection was conducted 11/20/2019. Evacuation drills have been conducted monthly; the last shelter in place drill was conducted 10/31/2019. Six children's files and two staff files were reviewed - please see Supplemental Summary for details. Violations:
Standard #: 22VAC40-185-40-G
Description: Based on review of six children's files, the facility failed to have documentation of viewing the proof of identity and age as required in 63.2-1809 in the Code of Virginia. Evidence: there was no proof of identity and age documented for Child 2 on file.

Plan of Correction: Center staff will request the proof of identity and age for each child within 7 days of enrollment.

Standard #: 22VAC40-185-200-A
Description: Based on review of staff records and staff interview, the facility failed to have a qualified director who is regularly on site at least 50% of the center's hours of operation. Evidence: the center could not provide documentation to support that staff meet qualifications of the position of director.

Plan of Correction: Two staff have enrolled in CDA classes to meet qualifications. An ad has been placed for the position of director.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not maintain the equipment inside the center in a clean and operable condition. Evidence: at approximately 12:00 p.m., the licensing inspector asked staff to plug in the portable sink in the infant room used for hand washing. The water that came out of the faucet was pinkish brown and had black residue in it. The water did not clear up after several minutes of running. Staff stated that it "usually" turns clear after running.

Plan of Correction: Center owner stated she will look into the issue with the water. In the mean time, staff can use the other changing table and sink.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of two new staff files, the facility failed to have documentation of an original fingerprint check for each staff. Evidence: the fingerprint check on file for Staff 1 (date of hire 8/26/2019) was from another child care facility dated 8/12/2019. The fingerprint check for Staff 2 (date of hire 9/3/2019) was from another child care facility dated 6/15/2018. The center did not conduct an original fingerprint check; fingerprints are not transferable.

Plan of Correction: Center director will schedule an appointment as soon as possible for Staff 1 and Staff 2 to get fingerprinted.
