Comments:
A Subsidy Health and Safety inspection was conducted on on Monday, December 2, 2019 between 11:20 am to 3:00 pm to review supplemental health and safety requirements. During this inspection, the center's director was present and assisted the licensing inspector throughout this inspection.The children were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 76 children and 19 staff present including administrative staff. The licensing inspector observed the children in care participating in a variety of activities to include eating lunch at the table, completing bathroom breaks/handwashing, transitioning into napping, sleeping in cribs, engaged in active play, diapering and feeding by staff, and napping. The center's classroom areas, bathroom, diapering stations, and equipment were inspected.The children's lunch today consisted of chicken patties, pork and beans, peaches, and milk. The first aid kit and emergency supplies, posted emergency contacts, fire and shelter-in-place drills were reviewed by the inspector. There were six children?s records and six staff's records reviewed during this inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-520-B-11
Description: Based on review of children's record and interview with staff, the center did not ensure to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: In Child#3's record (start date:2/15/19) there was no written allergy care plan found for diagnosed food allergy documented in physical. Interview with staff confirmed the allergy care plan for child was not obtain during the inspection.

Plan of Correction: Allergy care plan is in place now it was received on 12/04/19.

Standard #: 22VAC40-665-580-B
Description: Based on review of staff record and interview with staff, the center did not ensure all staff completed Virginia Preservice Training of Child Care Staff within the first 90 days of employment.

Evidence: In Staff#6's record (date of hire: 1/21/19) there was no certificate of completion found for Preservice training. Interview with staff confirmed the documentation was not available for review during the inspection.

Plan of Correction: VA Preservice training was completed just not in file. A copy is in file now.

Standard #: 22VAC40-665-580-F
Description: Based on review of staff records and interview with staff, the center did not ensure all staff who work directly with children completed CPR ad first aid training as required.

Evidence: In Staff # 4's record (date of hire: 07/25/19), there was no CPR and first aid training certification of completion found. Interview with staff confirmed documentation was not available for verification during the inspection.

Plan of Correction: CPR was looked at incorrectly for completion date. They are both in the files for staff.
