An unannounced monitoring inspection was conducted between the hours of 11:00 am-3:00pm. At the time of entrance, 100 children were in care with 18 staff members present. Children were observed during circle time, singing songs, reading books, completing art projects, and building with blocks. Handwashing and lunchtime was also observed. The children were served chicken nuggets, mixed vegetables, fruit, and milk. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. Evidence:
1. The record for Staff #1 did not contain a start date. Staff stated Staff #1 began employment on 10/10/19. The references documented in the record for Staff #1 were contacted on 10/16/19 and 10/17/19, after Staff #1's start date.Plan of Correction: The documentation of start date for Staff #1 was corrected during inspection.
Standard #: 22VAC40-185-550-M Description: Based on review of injury reports, 2 of 5 reports did not contain all required information.
Evidence:
2 of 5 injury reports reviewed did not contain the time parents were notified.

Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review and staff interview, an out of sate child abuse and neglect search was not completed for one staff.
Evidence:
Staff #1 began employment on 10/10/2019. Staff #1 has lived outside of the state of VA in the last 5 years. There was no documentation of an out of state child abuse and neglect search in the record for Staff #1. Staff stated an out of state child abuse and neglect search was not completed for Staff #1.Plan of Correction: The form was completed during the inspection and will be mailed out.
