Technical Assistance:
Discussed the concerns about keeping children's belonging separated and wires exposed on the floor.

Comments:
An unannounced monitoring inspection was conducted on May 31, 2019 from approximately 2:00 pm- 4:30 pm. Observations included programs, administration, supervision, special care provisions and emergencies, health and safety, and staff and children's records. Five staff records and five children's records were reviewed. The School Age children were observed playing games and art activity and talking freely in the teen room. There were 54 children present with five staff members. Additional staff were on site for administrative and support purposes. Exit interview was conducted with Staff#1.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, in one of six staff records reviewed, the licensee did not ensure that staff resubmit tuberculosis (TB) test results every two years.

Evidence: The most recent TB screening for Staff #6 was November 21, 2017. The TB screening was more than two years old

Plan of Correction: Staff #6 will get a TB test schedule in the next week.

Standard #: 22VAC40-185-60-A
Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information.

Evidence: As confirmed by Staff #1, the record for Child #2 did not have a complete address for both emergency contacts.

Plan of Correction: Staff #1 will talk to mom today to get complete addresses.

Standard #: 22VAC40-185-280-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:

1. The following unlocked hazardous substances were observed in the Teen Room that children had access to: a bottle of hand sanitizer and a spray bottle of a disinfectant.

2. All items were labeled keep out of reach of children and warning or caution

Plan of Correction: Staff will talk to the other programs share the space about licensing requirements for hazardous substances. Staff will make sure all hazardous substances locked up during the licensing program.

Standard #: 22VAC40-185-510-C
Description: Based on review of medication , the center did not ensure that procedures for out dated medication was followed.

:
Child #1 medication expired July 2019. The center did not follow there policy that states expired medication needs to be picked up with in 14 days. If medication is not picked up it will be disposed. Staff #1 confirmed the centers policy.

Plan of Correction: Staff #1 will give medication back to father today.
