An unannounced monitoring inspection was made on 12/16/2019. There were 12 children present with two staff supervising. Five children's records were reviewed today and two staff's records were reviewed on a previous date. No medications are being administered.
The inspector arrived at the school at 6:30 A.M. and departed at 8 A.M. The children were observed during arrival into the program, free play and various activities.

Standard #: 22VAC40-185-340-C Description: Based on observation, the center failed to ensure that during the stated hours of operation, there always shall be on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. The volunteer or other employee shall have received instruction in how to contact appropriate authorities if there is an emergency.
Evidence:
1. Upon arrival into the program at 6:30 A.M., staff # 1 was alone with four staff. There were janitors in the building but no means of direct communication between the staff and the janitor. The second staff person arrived at 6:57 A.M. there were six children present at that time.Plan of Correction: The staff will obtain phone numbers from the janitors to ensure direct means of communications. The staff will also make sure that two staff are present at all times when children are in care.
