63.2(17) License & Registration Procedures

Licensing Inspector arrived at 9:45 and departed at 11:30 AM. At the time of the inspection, there were 20 children present with five staff. Children were observed participating in circle time and free play

Standard #: 22VAC40-185-70-A Description: Based on review of three staff records, it was determined that the facility did not ensure that all required elements were present in each staff member's records.
Evidence:
The record for staff member #1 did not contain documentation of two references as to character & reputation being checked prior to employment and documentation of orientation.Plan of Correction: Center owner will ensure that references are obtained and that orientation is completed.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of three staff records, it was determined that the facility did not ensure that all staff members submitted to fingerprinting as required prior to employment.
Evidence:
Staff #1 (date of hire 12/16/19) was observed on duty with the children during the inspection. Although she had fingerprints on file, they were for another center.Plan of Correction: Center owner will send staff member today to get fingerprints.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of three staff records, it was determined that the facility did not ensure that if a staff member has lived outside of the state of Virginia within the past five years that a search of the central registry for that state is obtained.
Evidence:
The sworn statement or affirmation for staff #3 indicated that she had lived in Maryland within the past five years. A search of the central registry for Maryland had not been completed.Plan of Correction: Center owner will obtain a Maryland search of the central registry and place it in the records.
