Technical Assistance:
We discussed the requirements for infant formula and records to demonstrate required education and training. The risk assessments for violations were provided.

We reviewed the requirements for documenting injury notification, the maintenance of resilient surfacing, and use of the diaper disposal container.

Comments:
Thank you for your assistance during today's unannounced monitoring inspection conducted from 11:01 AM to 4:10 PM. Today, there were fifty-three children, ages infancy through four years, with ten staff. Six school age children were present before school. As many as fifteen school age children were expected to be supervised by one staff member after school. I viewed program activities, staffing, supervision, interactions with children, equipment, posted information, menu/nutrition, infant care and records, agency inspection reports, injury records, emergency supplies and drills, medication, five records for children, seven records for staff, staff qualifications, staff training, indoor and outdoor areas. There were age appropriate toys and equipment accessible to children. The teachers have planned season themed group activities. The staff were seen providing attentive supervision and frequent interactions with the children in their care.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of seven records for staff, and an interview with administrative staff, the records of staff member 2 and 5 did not include all required information. Evidence: The record of staff member 2 did not include college transcripts to demonstrate the education qualification requirements for the position held was met. The record of staff member 5 only contained documentation of nine of a minimum of twenty-four hours of training in only three of the required topics for a qualification requirement. There was a test documented from the completion of studies as a vocation student. However, the hours of training during coursework and the required topics of child development and child abuse/neglect reporting were not documented for the record.

Plan of Correction: The director will arrange for staff member 2 to submit all college transcripts to demonstrate a minimum of twelve semester hours were completed in child related studies. Also, the director will arrange for staff member 5 to obtain written information on the hours and topics of coursework completed.

Standard #: 22VAC40-185-570-I
Description: Based on a review of infant formula for children, and an interview with staff, the center did not ensure that there was at least a one day's emergency supply of commercial formula appropriate for all children in care. Evidence: Child 1F had only enough powdered formula for one feeding stored at the center.

Plan of Correction: The teacher will arrange for the parent to provide enough extra formula for at least a day's emergency supply. The director will review the requirements for extra formula with the infant caregivers.
