An unannounced monitoring inspection was conducted on 12/19/2019. There were 44 children in care with 13 staff directly supervising the children during the inspection. Prior violations from the previous inspection were reviewed. A physical plant tour was done inside and outside. The inspectors arrived at 7:30 A.M. and departed the program a 10:30 AM. The inspector reviewed five children's records during the inspection and seven staff's record.
Discussion with director about the transportation, TB screening

Standard #: 22VAC40-185-160-C Description: Based on review of seven staff's records, the center failed to ensure that TB test and or screening every two years.
Evidence:
1. Staff # 5 most recent TB was 6/17/2017, staff # 6 most recent TB was 10/6/2017 and staff # 7 most recent TB was 10/6/2017.Plan of Correction: The director stated that she thought the TB were every three years. The director will have the staff to complete the TB ASAP.
Standard #: 22VAC40-185-500-B Description: Based on observation, the center failed to ensure that all diapering requirements were met.
Evidence:
1. In the toddler's and two year old rooms the trash can used for diapering did not allow for disposal with out the diaper or the staff's hand to touch the lid.Plan of Correction: The director will purchase new trash cans with a foot operated opening.
