Comments:

The licensing inspector conducted an unannounced renewal inspection on 12/17/2019 from 10:04am to 3:15pm. The children were observed learning through play in centers, eating lunch and preparing for nap. The infants were observed eating, having tummy time and floor play. A formal observation was completed with the toddler and two year old classrooms. The lunch menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms were inspected today as well as the facility?s playground. Five children?s records and ten employee records were reviewed during this inspection. Medication is not being administered. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 12/05/2019 Last shelter-in-place drill: 09/19/2019 Last fire inspection: 09/24/2019 Last health inspection: 09/05/2019 Today, the following child to staff ratios were observed: Infants ? 9:3 Toddlers ? 5:1 Twos ? 12:2 Preschool 1- 13:2 Preschool 2- 8:1 Jr. Kindergarten ? 16:2 The violations from the previous inspections were checked for corrections. Description:	Based record review, the center did not ensure three of five children's records had documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The record of child #2 (start date: 1/21/19) did not have documentation of additional immunizations. The most recently received immunization record was dated 12/20/2018. 2. The record of child #4 (start date: 10/15/18) did not have documentation of additional immunizations. The most recently received immunization record was dated 3/18/2019. 3. The record of child #5 (start date: 4/8/19) did not have documentation of additional immunizations. The most recently received immunization record was dated 3/26/2019.
Plan of Correction:	The center ensures that children's records have documentation of additional immunizations once every 6 months for children under the age of 2 years.

Description:	Based on record review, the center did not ensure that one of six staff records had documentation of an out of state child abuse and neglect search request by the end of the 30th day of employment. Evidence: 1. The record of staff #6 (start date 1/30/19) did not have documentation of an out of state child abuse and neglect search request. The staff's record indicated that the staff lived out of the state of Virginia within five years prior to the date of hire. 2. The record of staff #7 (start date 9/30/19) did not have documentation of an out of state child abuse and neglect search request. The staff's record indicated that the staff lived out of the state of Virginia within five years prior to the date of hire.
