An unannounced subsidy health and safety inspection was conducted on 01/03/2020 from 10:45 am - 1:15 pm. The inspector returned on 01/07/2020 to complete the inspection which began at 3:25 pm and concluded at 4:45 pm.
A renewal inspection was concurrently conducted as the facility is a licensed child day center.
Records were reviewed for six staff and ten children on 01/03/2020.
There were 70 school age children in care with eight staff and the program director on 01/07/2020. The inspector reviewed transportation procedures, program, activities and afternoon snack which consisted of fresh mixed berries and graham crackers.

Standard #: 22VAC40-665-540-A Description: Based on record review, the vendor failed to ensure that the required TB screening shall have been completed within 30 calendar days of employment.
Evidence:
1. Staff 1 has a hire date of 03/18/ 2019 and the TB screening was conducted on 03/20/2019.
2. Staff 2 has a hire date of 02/04/2019 and the TB screening was conducted on 02/09/2019.Plan of Correction: All staff are now required to submit a TB screening that has been completed no more than 30 days prior to employment.
