Comments:
An unannounced renewal inspection was conducted on December 2, 2019. Additional documentation was reviewed at the administrative office, N-Deck on December 11, 2019. The inspector was on site from approximately 10:30AM to 2:30PM. Children were observed engaged in a variety of activities during the inspection. Activities included listening to the story "Come Cook with Me", preparing for a cooking project, circle time where children participated with a flannel board story and sang songs, played with toys, and had outdoor time on the playground with balls and small trikes. Infants were observed playing with balls, napping and playing on the floor with staff. Diapering and hand washing was also observed. Lunch was served during the inspection. All areas of the facility including classrooms, hallways, bathrooms and playgrounds were inspected. The facility was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports, daily attendance, daily sheets, infant feeding plans, menu, license, emergency numbers, daily schedules, and evacuation maps/procedures. The inspector interacted with staff and children in care.

Today's census observed:
Infants (Meadow Room): 8 children and 2 staff
Older Infants (Rapids Room): 8 children and 2 staff
Toddlers (Waterfall Room): 9 children and 2 staff
Twos (Stream Room): 13 children and 2 staff
Threes (Babbling Brook Room): 8 children and 2 staff
Fours and Fives (River Room): 11 children and 2 staff

During the inspection, eight children's records and eight staff records were reviewed. Staff have current CPR, First aid and DHO training. The most recent evacuation drill was documented on 11/18/2019. The two required shelter-in-place drills were completed on 04/24/2019 and 10/15/2019. Standard #: 22VAC40-185-130-A
Description: Based on review of children's records, the center did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child attends the center.

Evidence:

The record of child #8, enrolled 12/17/2018, had documentation of immunizations dated 03/22/2019, exceeding the first day of attendance.

Plan of Correction: Per the administration: We will ensure all required documentation is obtain at the time of enrollment for each child.

Standard #: 22VAC40-185-140-A
Description: Based on review of children's records, the center did not obtain documentation of a physical examination before the child's attendance or within one month after attendance.

Evidence:

The record of child #8, enrolled 12/17/2018, had documentation of a physical dated 03/22/2019, exceeding one month from the first day of attendance.

Plan of Correction: Per administration: We will ensure all required paperwork is obtained at the time of enrollment.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff records, the center did not obtain all required information for each staff record.

Evidence:

The record of staff #1 and staff #4, did not contain the address for a person to be notified in an emergency which shall be kept at the center.

Plan of Correction: Per the administration: We will have staff complete the missing information and maintain a copy on site.

Standard #: 22VAC40-185-280-G
Description: Based on observation and interview, the center did not ensure when hazardous substances are not kept in the original containers, the substitute containers was clearly labeled with their contents.

Evidence:

1. A spray bottle was observed on the counter top in the older infant classroom. The spray bottle was filed with a liquid and was not labeled as to what that liquid was.
2. Staff identified the liquid as soap and water.

Plan of Correction: Per the staff member: I will label the spray bottle.

Description: Based on observation and interview, the center did not ensure the findings of the most recent inspection of the facility were posted.

Evidence:

1. The findings of the most recent inspection of the facility were not posted.
2. The assistant director acknowledged the findings of the most recent inspection of the facility were not posted.
