Comments:
An unannounced monitoring inspection was conducted Tuesday, December 26, 2019 to determine the facilities compliance with licensing standards. The inspection was initiated at 10:00 am and concluded at 1:00 pm. The licensing inspector observed 29 children being supervised by seven staff members. Today the center was offering full day car. Children were observed playing in centers that consisted of blocks, books, games and coloring. Later children played basketball in the gym. A field trip was schedule for later in the day to see the movie "Spies in Disguise". Children provided their own lunch for the day. The center provided a snack of mini bagels, cream cheese and celery. Hand washing was conducted before meals. The center was equipped with toys and supplies and items were available to the children. The first aid kit and non-medical emergency supplies were inspected and found complete. The following information was reviewed: injury reports, daily attendance sheets, menu, injury prevention plan, required postings and parent agreements. The inspector interacted with children and staff.

Emergency contact and health information for seven staff was reviewed along with ten children's records.
Medication, authorizations and care plans were reviewed. Staff on site have current CPR, First aid, DHO and MAT training.
The most recent evacuation drill was documented on 11/27/2019, the two required shelter-in-place drill were completed on 09/17/2019 and 07/20/2019. A lock down drill has not yet been conducted for this year.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on review of staff records and interview, the center did not maintain the name, address and telephone number of a person to be notified in an emergency which shall be kept at the center for each staff record.

Evidence:

1. Staff #4, #5, #6 and #7, all on site and working, did not ave documentation of a name, address and phone number of a person to be notified in an emergency that was kept on site at the center.
2. The director stated she did not have the emergency contact information for staff #4, #5, #6 or #7 on site.
3. The record of staff #8, hired 04/05/2019, did not have written documentation of orientation training in the record.
4. Administration stated orientation was conducted but not documented.

Plan of Correction: Per the director: I will have each site bring binders and make copies of the emergency contact information.

Standard #: 22VAC40-185-260-A
Description: Based on review of documentation, the center did not obtain an annual fire inspection report from the appropriate fire official having jurisdiction.

Evidence:

The most recent fire inspection report was dated 11/09/2018, exceeding the annual requirement.

Plan of Correction: Per the director: I have contacted multiply times with no response. I will try again after the first of the year.

Standard #: 22VAC40-185-260-B
Description: Based on review of documentation, the center did not obtain an annual approval from the health department.

Evidence:

The most recent approval from the health department was dated 11/19/2018, exceeding the annual requirement.

Plan of Correction: Per the director: I have contacted multiple times with no response. I will try again after the first of the year.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement, the center did not ensure where playground equipment is provided, resilient surfacing complied with the minimum safety standards.

Evidence:

The rear fall zone of the swing set did not contain resilient surfacing beyond four feet of the edge of the swing set to include the child's trajectory in the event of a fall.

Plan of Correction: Per the director: I will contact the operations and branch for a solution. Children will not play on the swings until additional mulch is added.
