Comments:
An announced initial monitoring inspection was conducted between 8:00 am and 12:00 pm. A tour was conducted of the facility, space was calculated,and staff records reviewed.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center were not maintained in a safe condition.
Evidence:
1. There was a wooden parking block in front of the center's entrance that was splintered and jagged at the crack.
2. On the playground there was a tricycle with rusted handles and rusted hardware on the playhouse.

Plan of Correction: The following corrective action was provided by the owner during the exit interview:
The tricycle will be discarded and replaced and new hardware will be placed on the playhouse. The parking block will be removed.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement the center did not have the required resilient surfacing where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99.
Evidence:
There were three Little Tyke slides on the playground that had about 3 inches of resilient surfacing in the fall zones, where 6 inches is required.

Plan of Correction: The following corrective action was provided by the owner during the exit interview:
The resilient surfacing area will be widened to at least 6 feet around the equipment.
