Comments:
An unannounced monitoring inspection was conducted on Tuesday, December 10, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 9:30 AM and concluded the inspection at 1:15 PM. The census for today's inspection consisted of 104 children. During the inspection, children were making gingerbread people out of playdough, making castles in the block area, coloring, washing hands, eating snack, exploring the sensory tables, playing on the playground and preparing for departure. Children's equipment, learning materials, indoor and outdoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed. The provider stated transportation is not provided at this time. Medication is administered and medication, authorization forms and allergy plans were reviewed. Ten children's records and ten staff records, annual fire and health inspections, fire drill and shelter in place drill logs, daily attendance and all required postings were reviewed.

The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that all hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In classroom #1, one bottle of liquid hand soap labeled "Keep out of reach of children"; one container of concentrated room deodorizer labeled ?Caution? and "Keep out of reach of children" was on a shelf accessible to the children.
2. In classroom #2, one bottle of Windex labeled "Keep out of reach of children" was in an unlocked cabinet accessible to the children.
3. In classroom #3, one container of Clorox disinfecting wipes labeled "Keep out of reach" and "Danger" was in an unlocked cabinet accessible to the children.
4. In classroom #4, one container of shaving cream labeled "Keep out of reach of children".

Plan of Correction: 1. Child safety cabinet locks added
2. Teacher reminders and training planned

Standard #: 22VAC40-191-40-D-6
Description: Based on a staff record review and interview, the center did not ensure that a required criminal history record report or a central registry finding from an applicant required to obtain background checks is not dated more than 90 days prior to the date of employment.

Evidence:
1. The record for staff #3 (date of hire 08/26/19) had documentation of fingerprint-based national criminal background check dated 02/01/2019 and a documentation of central registry findings dated 02/28/2019.
2. The record for staff #4 (date of hire 08/26/2019) had documentation of fingerprint-based national criminal background check dated 09/04/2018 and a documentation of central registry findings dated 10/22/2017.
3. The record for staff #6 (date of hire 08/26/2019) had documentation of fingerprint-based national criminal background check dated 07/10/2018 and a documentation of central registry findings dated 02/26/2018.
