Standard #: 22VAC40-185-70-A Description: Based on record review and verification from administrator, there was not written information to
demonstrate that the individual staff member possesses the education and experience required by the
job position.
Evidence:
1. Reviewing staff #1, staff #2, staff #3, and staff #6?s records there was no written documentation
of education or job history.
2. Administrative staff verified that there was not written documentation or an application on file
that had that information.Plan of Correction: All current staff will be required to provide documentation of education and job history to be added to their file. In the future all applicants will be given an application that includes questions regarding education and job history.
Standard #: 22VAC40-185-540-C Description: Based on the review of the emergency supplies and verification from administrator, the facility failed to
maintain the minimum requirements of a first aid kit.
Evidence:
1. The first aid kits did not contain antiseptic cleansing solution/pads.
2. Administrative staff stated that they had taken the antiseptic cleansing solution/pads out of the
first aid kits.Plan of Correction: Antiseptic cleansing solutions/pads will be purchased and added to each of the first aid kits.
