Approved Subsidy Vendors are required to notify the department of the occurrence of an Injury Requiring Outside Medical Treatment (IROMT) as soon as practicable, but not to exceed two (2) business days of the occurrence. Serious injury means an injury of a child under the care of the center that requires outside medical treatment. Reports can be submitted online at:

Comments:
An unannounced subsidy complaint inspection was conducted with a local agency representative on December 20, 2019 in response to a complaint regarding a serious injury that occurred at the center. Based on staff interviews and documentation, the facility failed to report a serious injury that required outside medical treatment to the Division within two business days. Description: Based on interviews and documentation review, the center did not notify the department within two business days of a serious injury to a child - while under the center's supervision - that required outside medical attention. Evidence: A child sustained an injury to the face while in the center's care on 12/17/2019. The parents took the child to the hospital for examination on 12/17/2019 and advised the center of the visit on 12/18/2019. The center did not report the injury to the licensing inspector or complete an Injury Requiring Outside Medical Treatment (IROMT) online.

Plan of Correction: The center will complete an IROMT online whenever there is an injury while being supervised by the center that requires outside medical treatment.
