Technical Assistance:
Today we discussed the following standards: posting the license, meals and outside play.

Comments:
An unannounced monitoring inspection was conducted today from 9:30am through 12:30pm. There were 20 children in care ages 20 months through eight years old. There were five staff present. Children and staff records were reviewed. Children were observed in classroom activities both teacher and self-directed, outside play lunch and nap. Violations regarding staff records, unlocked chemicals and unlocked medication were identified and appear on the violation notice. These violations were discussed with the owner at the conclusion of the inspection.

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview the center failed to ensure that each staff record contains all required information.
Evidence:
1 - The record for staff 2 does not contain documentation of a date of hire or education.
2 - The record for staff 3 does not contain documentation of date of hire, experience nor references.
3 - The record for staff 4 does not contain documentation of date of hire nor required experience.
4 - The record for staff 5 does not contain documentation of date of hire.
5 - The record for staff 6 does not contain documentation of date of hire, emergency contact, education nor date of separation from service.
6 - Staff 1 confirmed that the records for staff 2 - 5 do not contain all required information.

Standard #: 22VAC40-185-280-B
Description: Based on observation and interview the center failed to ensure ensure that all hazardous substances are kept in a locked place.
Evidence:
1 - There is an unlocked, wall mounted cabinet in the preschool room where there is one spray can of surface cleaner/sanitizer and 1 pump bottle of hand sanitizer.
a. The chemicals in the cabinet are accessible to children in care.
b.The label on these items reads keep out of reach of children and caution, hazardous.
2 - Staff 2 stated that she had unlocked the cabinet but not locked it back when she was finished.

Description: Based on observations and interview the center failed to ensure that cleaning and sanitizing materials were not stored in areas physically separate from food.
Evidence:
1 - In the preschool classroom in a wall mounted cabinet there is a box of crackers next to spray can of surface sanitizer and hand sanitizer.
2 - Staff 2 confirmed that there were crackers stored in the cabinet next to cleaning chemicals.

Description: Based on observation the center failed to ensure that all is stored in a locked place.
Evidence:
1 - There is unlocked medication stored in a wall mounted cabinet in the preschool classroom. Children present in the classroom range in age from four to eight years old. At least one child is tall enough to reach the medication in the cabinet.
a. The medication is in a cabinet approximately five feet above the floor.
b. The cabinet contains aspirin, non-aspirin and antacid medications.
2 - Staff 2 confirmed that there was medication in the unlocked cabinet.

Description: The center failed to ensure that each first aid kit was easily accessible to staff but not to children.
Evidence:
1 - There is a first aid kit in the preschool classroom that is within reach of children in care.
2 - Staff 2 stated that she had unlocked the cabinet jut had not re-engaged the lock.
