Comments:
An unannounced monitoring visit and facility tour was conducted from 11:25 am-12:30 pm. At the time of inspectors' arrival there were 26 pre-school and school aged children in care with 6 staff members. Children were observed participating in indoor play, transitioning between activities, lunch, and arrival. A sample of 5 children's records and 5 staff on-site records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review it was determined the center did not ensure that each child's record contained all of the required information.
Evidence:
1. The record for child #2(enrolled 9/3/19) did not contain the addresses for the two required emergency contacts.
2. The record for child #4(enrolled 1/13/20) did not contain the address for one of the two required emergency contacts and the father's address.

Plan of Correction: -

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. The chairs in classroom #1, 2, and 3 had tears in the outer covering which exposed the foam core.
2. In classroom #3 there large LEGO base plates on the LEGO table were broken or cracked.
3. The above noted areas were accessible to children in care during the inspection.

Plan of Correction: -

Standard #: 22VAC40-185-280-B
Description: Based on observation and interview it was determined the center did not ensure all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In classroom #1 in an unlocked upper cabinet there was Lysol, Clorox wipes, sanitizer, and other cleaning agents.
2. Staff #5 stated that the cabinet the chemicals were in was difficult to lock.

Plan of Correction: -

Standard #: 22VAC40-185-560-G
Description: Based on observation it as determined the center did not ensure that when food is brought from home it is labeled with the name of the owner and dated.
Evidence:
In classroom #1 6 of the 11 lunch boxes were not properly dated.
