An unannounced monitoring visit and facility tour was conducted from 10:15 am-11:30 am. At the time of inspectors' arrival there were 19 preschool aged children in care with 3 staff members and 1 administrator. Children were observed participating in free play, interacting with caregivers, hand washing, and lunch. A sample of 5 children's records and 4 staff records were reviewed. The Director reported that no medications are given at the center at this time. Areas of noncompliance are detailed on the Violation Notice and were discussed with the Director at the conclusion of this inspection.

Standard #: 22VAC40-185-270-A Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. There was broken plaster/drywall and peeling paint in the Green classroom.
2. There was peeling paint in the Orange classroom.
3. There was peeling paint, an electrical plug with exposed metal prongs partially inserted in an electrical outlet, and a space heater on a upper shelf with a cord dangling down in the Blue classroom.
4. Theses areas/items were all accessible to children in care during this inspection.Plan of Correction: The electrical plug and dangling cord were corrected during this inspection. Paint and plaster will be repaired.
