Technical Assistance:
Provided consultation on:
1) standard 22 VAC 40-185-160B: Acceptable forms of documentation of a negative tuberculosis (TB) screening are 1. A clearance statement signed by a physician, the physician's designee or an official of the local health department; 2. The results of a negative tuberculin skin test (TST); or 3. The results of a chest x-ray negative for tuberculosis disease.
2) 22 VAC 40-185-70A: staff records shall contain the name, address and telephone number of a person to be notified in an emergency

Comments:
An announced, focused, monitoring inspection was conducted from 9:30 -11:30 am offsite at the Ron Rosner YMCA office. Two staff records were reviewed. If you have any questions about this inspection, contact Donna Liberman at 540-359-5244 or Donna.Liberman@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of two staff records, not all staff had a repeat tuberculosis (TB) test/screening at least every two years from the date of the previous test/screening. Evidence: the record for staff #2 had a TB test/screening dated 2/24/17.

Plan of Correction: Staff will obtain an updated TB test/screening.

Standard #: 22VAC40-185-70-A
Description: Based on review of two staff records, the facility did not ensure that each staff record contained all required information. Evidence: the record for staff #2 did not have written information to demonstrate that the individual possesses the education, staff development, certification, and experience required by the job position.

Plan of Correction: We will request from staff documentation of lead teacher qualifications and add it to their files. Moving forward, we will ensure staff hired as a PL2 will have lead teacher qualifications on file.
