Provided consultation on:
1) standard 22 VAC 40-185-160B: Acceptable forms of documentation of a negative tuberculosis (TB) screening are 1. A clearance statement signed by a physician, the physician's designee or an official of the local health department; 2. The results of a negative tuberculin skin test (TST); or 3. The results of a chest x-ray negative for tuberculosis disease.
2) 22 VAC 40-185-70A: staff records shall contain the name, address and telephone number of a person to be notified in an emergency

An announced focused monitoring inspection was conducted from 1:00 - 4:30 pm offsite at the Massad central office. Four staff records were reviewed and in compliance.
