Technical Assistance:
We discussed the requirements for infants fed formula. The risk assessments for violations were provided.

We reviewed the requirements for infant and toddler nap time accommodations and playground safety procedures.

Comments:
Thank you for your assistance during today's unannounced renewal inspection conducted from 11:22 AM to 4:17 PM. Today, there were sixty-one children, ages infancy through five years, with eleven staff. I viewed staffing, supervision, interactions with children, equipment, posted information, infant care and records, nutrition, menu, daily routines, diapering procedures, program activities, injury records, emergency supplies and drill records, medication, agency inspection reports, six records for children, thirteen staff records, staff qualification requirements, staff training, indoor and outdoor areas. The children have enjoyed outside experiences, weather permitting. Activities have been planned according to themes such as animals. Age appropriate toys, materials and play equipment were available for each grouping of children.

Standard #: 22VAC40-185-570-C
Description: Based on a review of records for each child on formula, and interviews with staff, the records regarding the feeding of formula did not include all required information. Evidence: The forms used for indicating the brand of formula for child 1F and 2F did not include written information from the parents regarding each child's feeding schedule.

Plan of Correction: The infant teacher gave a form to each parent at pickup today and asked for the feeding schedule. The infant staff will make sure each child who will be fed formula at the center has written information from parents that includes both the brand and the feeding schedule. As needed, administrative staff will work with the teacher to make sure all information has been maintained for each infant.

Description: Based on a review of prepared infant formula, and an interview with staff, the center failed to ensure that prepared infant formula brought to the center was labeled as required. Evidence: Two bottles of formula viewed in the refrigerator for child 2F were not dated. An empty bottle shown to the inspector that was used to feed child 2F earlier this date was missing a date. Formula for other infants either had dates written on the vinyl name bands or had stickers added with today's date.

Plan of Correction: The infant teacher will talk to the parent about the need to date the bottles of formula brought to the center. Each day, the infant room staff will view the bottles placed in the refrigerator to make sure the bottles are labeled as required.
