An announced Subsidy Health and Safety Inspection (SHSI) staff record review was conducted today at the Ron Rosner YMCA Administration Office from 9:30am to 12:30pm. Staff records were reviewed for documentation of TB test, CPR and First Aid certification, as well as the mandatory Virginia Preservice Training. An exit interview was conducted with the Manager on 1/31/20. Standard #: 22VAC40-665-540-A Description: Based on review of three staff records, the facility did not ensure that staff were evaluated by a health professional and issued a statement that the individual was determined to be free of communicable tuberculosis (TB) and that documentation of the screening was submitted at the time of employment and prior to coming into contact with children. Evidence: The TB test or screening for Staff A, with a start date of 1/2/20, was dated 1/7/20. Plan of Correction: Moving forward managers will ensure that staff assigned to a subsidy site submit their proof of a tb test or assessment screening prior to start.
