Technical assistance was provided regarding staff emergency contacts kept at the center. The emergency contact shall include the name, address and telephone number of a person to be notified in an emergency for each staff. In addition, technical assistance was also provided regarding acceptable forms of documentation of tuberculosis screening. Documentation shall include at least one of the following: (1) A clearance statement signed by a physician, the physician's designee or an official of the local health department, (2) The results of a negative tuberculin skin test (TST). The documentation shall include the date the test was given and results of the test and be signed by a physician, physician's designee or an official of the local health department, or (3) The results of a chest x-ray negative for active tuberculosis disease.

An announced non-mandated focused inspection was conducted 1/16/2020 between the hours of 1:30pm through 4:30pm at Massad Family YMCA located in Falmouth, Virgina. A total of 4 staff files were reviewed and violations are documented in the Violation Notice. The exit interview was completed 1/31/2020 with the YMCA Manager.

Evidence: Staff #2 (Start Date: 9/7/19) did not have central registry findings on file within 30 days of employment.Plan of Correction: Central registry findings could not be located at the time of inspection, however, the results page was later located and was found to be dated 10/17/2019.
