Technical Assistance:
Technical assistance was provided regarding staff emergency contacts kept at the center. The emergency contact shall include the name, address and telephone number of a person to be notified in an emergency for each staff. In addition, technical assistance was also provided regarding acceptable forms of documentation of tuberculosis screening. Documentation shall include at least one of the following: (1) A clearance statement signed by a physician, the physician's designee or an official of the local health department, (2) The results of a negative tuberculin skin test (TST). The documentation shall include the date the test was given and results of the test and be signed by a physician, physician's designee or an official of the local health department, or (3) The results of a chest x-ray negative for active tuberculosis disease.

Comments:
An announced non-mandated focused inspection was conducted 1/16/2020 between the hours of 9:30am through 12:30pm at Ron Rosner Family YMCA of Spotsylvania County located in Fredericksburg, Virginia. A total of 2 staff files were reviewed and violations are documented in the Violation Notice. The exit interview was completed 1/31/2020 with the YMCA Manager. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, 1 of 2 staff records reviewed did not contain documentation of a negative tuberculosis screening that was completed no later than 21 days after employment.

Evidence: Staff #2 (Start Date: 11/5/19) has negative tuberculosis results on file dated 1/10/20.

Plan of Correction: Moving forward, I will make sure we have TB tests within 21 days and they will be repeated every 2 years.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, 1 of 2 staff records did not contain central registry findings within 30 days of employment.

Evidence: Staff #2 (Start Date: 11/5/19) has documentation on file stating the central registry search request was sent 12/18/19 and no results were on file.

Plan of Correction: We sent the request on 12/18/19 and followed up with them and we were told the results were not sent out. We received the central registry results on 1/23/2020 and they are now on file, however, the staff's last day employed by YMCA was 1/16/2020.
