Technical Assistance:
Standard 570.C - documentation of infant feeding schedules discussed.

Comments:
An unannounced renewal inspection was conducted on 01/29/2020 from 12:35 pm - 2:35 pm. At the time of entrance there were 66 children in care with 17 staff. The children ranged in age from infant - four years. Children were observed during nap. Diaper changing was observed in the infant room. Records were reviewed for 11 children in care. Medication and emergency supplies were additionally reviewed. Eight staff records were reviewed at the Children's Center HR office from 3:05 pm - 3:40 pm.
Violations were observed in four parts of the CDC standards and are listed on the violation notice.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review and interview, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening.

Evidence:
1. There was no documentation of a tuberculosis screening for staff 1 who has an employment date of 07/18/2006.
2. Administrative staff stated that they could not locate documentation of a tuberculosis screening for staff 1.

Plan of Correction: Staff 1 tuberculosis screening has been obtained and submitted to H.R.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center shall be maintained in a safe condition.

Evidence:
1. The lock on the top dutch door to the children's bathroom in room 107 was loose resulting in a pinch point when the door was opened.
2. Five areas of peeling paint were observed on the wall in the children's bathroom in room 103. These areas were within reach of children using the toilet.

Plan of Correction: 1. A work order has been requested for new lock to be put on dutch door classroom 107.

2. Bathroom walls in classroom 104 have been scrapped and sanded. Request for walls to be repainted has been submitted to maintenance.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the center failed to ensure that when food is brought from home the food container shall be sealed and clearly dated and labeled in a way that identifies the owner.

Evidence:
1. Two open containers of powdered infant formula was not dated and labeled with the child's name.
a. Staff in the infant room indicated that both formula's were provided by the parent's of two different children in care.

Plan of Correction: All containers of formula in infant room have been labeled with child's name and dated.
Staff will label and date containers of formula as soon as they are opened.

Description: Based on observation, the center failed to post the findings of the most recent inspection of the facility.

Evidence:
1. The findings from the most recent inspection conducted on 10/23/2019 were not posted in the facility.
a. The findings from the inspection conducted on 04/10/2019 were posted.

Plan of Correction: Recent inspection has been posted in the center. Site Supervisor will ensure all inspections are posted as soon as they are received.
