Evidence:
1 - Child record A does not include: the parent's home phone number; documentation that the parent has reviewed the child's contact information within the past year.
2 - Child record B does not include: the parent's home phone number; the mother's work phone number.
3 - Child record C does not include: "authorized pick ups" information; documentation that the parent has reviewed their child's contact information within the past year.
4 - Child record D does not include: documentation that the parent has reviewed their child's contact information within the past year.
5 - Child record E does not include: the parent's home phone number.Plan of Correction: All parents will be asked for the missing information.

Evidence:
Containers of table cleaning wipes were stored inside unlocked cabinets in the Wise Owls classroom and the Busy Bees classroom.Plan of Correction: They will be locked today.
Standard #: 22VAC40-185-280-G Description: Based on observations, it was determined that two substitute containers of potentially hazardous substances were not labeled to indicate the contents.
Evidence:
In the Teddy Bears classroom and the Busy Bees classroom, the spray bottles of bleach/water cleaning solution were not labeled.Plan of Correction: They will be labeled today.
Standard #: 22VAC40-185-510-A Description: Based on a review of children's medications, it was determined that school has not obtained written authorization for one child's long term medication.
Evidence:
The authorization form for one child's long term emergency medication has not been signed by the parent.Plan of Correction: I will ask the parent to sign the form today.
