Technical Assistance:
Questions regarding the standard requiring updated immunization records every six months for children under the age of two years were discussed with the program director.

Comments:
An unannounced, mandated renewal inspection was conducted today. The inspector was in the facility from 10:30 AM until 1:30 PM. Eighteen children were in care. Morning activities, lunch service and nap time were observed. A sample size of five staff records and five children's records were reviewed. It was reported that no medication has been administered to children in care since the previous inspection. No children's injuries were reported since the previous inspection.
An exit meeting was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based upon observation and staff interview, the facility has not ensured that equipment is maintained to be clean.
Evidence:
1. The cloth covers on two infant seats in the infant classroom are dirty and stained.
2. Infant staff verified that the covers were dirty and could be removed to be cleaned.

Plan of Correction: The facility responded with the following:
The covers were removed and placed in the washing machine during the inspection. Once clean, the covers and bouncy seats will be stored until needed if a young infant enrolls. All staff will be reminded that when equipment becomes dirty it must be washed/sanitized.

Standard #: 22VAC40-185-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in locked locations using a safe locking method that prevents access by the children.
Evidence:
Upon arrival of the inspector, children were in the open classroom. The door to the kitchen directly adjacent to this classroom was not secured with the child proof latch. The child proof latch on the left cupboard under the sink no longer catches. There was a container of Lysol with a hazard warning label in the cabinet.

Plan of Correction: The facility responded with the following:
All staff will be reminded that the latch on the door to the kitchen must be latched at all times the kitchen is unattended. The child proof latch on the cupboard under the sink will be replaced.

Standard #: 22VAC40-185-560-F
Description: Based upon observation, the facility has not ensured that when meals are provided, children are allowed second helpings of food listed in the USDA's child and adult care meal patterns.
Evidence:
The infant and toddler children were observed during lunch. Each child was served two fish sticks, french fries, milk and apple sauce. None of the children were offered or asked if they wanted seconds of any of the food served. Dishes were portioned out and brought into the lunch rooms. Additional food items were not visible or available to the staff serving the lunches.

Plan of Correction: The facility responded with the following:
Staff are supposed to ask the children if they would like second helpings. Second helpings of food being served will be provided to the staff serving the children in the lunch room.

Standard #: 22VAC40-185-560-G
Description: Based upon observation and staff interview, the facility has not ensured that when food is brought from home, the food container is clearly dated and labeled in such a way that identifies the child to whom it belongs.
Evidence:
1. There was a lunch box with foods and drinks inside on top of the cubbies outside the upstairs preschool classroom. Neither the lunch box or food containers were labeled with the date or the child's name.
2. Staff 5 verified that the lunch box/food from home belonged to child 5 and that it was not labeled.

Plan of Correction: The facility responded with the following:
Staff will be reminded that if children bring in food from home, it must be labeled with the date and the child's full name.
