Technical Assistance:
Posting of children's allergies, dietary preferences was discussed with the program director.

Comments:
A Subsidy Health and Safety inspection was conducted on 02/10/2020 to review supplemental health and safety requirements. The inspector arrived at 10:15 AM and departed at 1:30 PM. Forty-six children were in care. A sample size of five staff records and five children's records were reviewed.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based upon review of five staff records and staff interview, the facility has not ensured that documentation of a tuberculosis screening is submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence:
1. The record provided for staff 5 did not include documentation of a tuberculosis screening. According to the program director, staff 5 began working with the children on 02/07/2020.
2. The program director acknowledged that results of a tuberculosis screening have not been obtained for staff 5.

Plan of Correction: The facility responded with the following:
In the future, results of tuberculosis screenings will be obtained within the required time frame and before the new staff is with the children.
