Comments:

An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 10:15 Am and departed at 2:00 PM. Forty six children were in care. Morning activities, lunch service and nap time were observed. A sample size of five staff records and five children's records were reviewed. It was reported that no medication has been administered to children in care since the previous inspection. An injury record was reviewed. An exit meeting was conducted with the program director prior to closure of the inspection.

Description:	Based upon review of five staff records and staff interview, the facility has not ensured that tuberculosis screening results are obtained every two years. Evidence: 1. The most recent documentation of a tuberculosis screening in the record of staff 1 is dated 1/26/2018. 2. The program director verified that an updated tuberculosis screening had not been obtained for staff 1.
Plan of Correction:	The facility responded with the following: All staff will obtain tuberculosis screening every two years.

Description:	Based upon review of five staff records and staff interview, the facility has not ensured that each staff record includes all required documentation. Evidence: 1. The record provided for staff 4 did not include any documentation that references as to character, reputation and qualifications were obtained. Staff 4 was hired 11/18/2019. 2. The record provided for staff 4 did not include documentation of the required training prior to the end of staff 4's first day of employ with the children. 3. The record provided for staff 5 included only one of the required two reference checks as to character, reputation and qualifications prior to hire on 2/7/2020. 4. The record provided for staff 5 did not include the address of the emergency contact person.. 5. Staff 5 began employ on 2/7/2020. The record provided did not contain documentation of the required training prior to the end of the first day of employ with the children. 6. The program director verified that the above documentation was not in the records provided for staff 4 and staff 5.
Plan of Correction:	The facility responded with the following: All staff records will be reviewed to ensure that all required documentation is included in a timely fashion. Reference checks will be obtained and documented prior to employment. Orientation training will be documented by the end of the first day of employment.

Description:	Based upon observation, the facility has not ensured that there is a posted daily schedule in the toddler and preschool classrooms. Evidence: There was no posted daily schedule in the three year old classroom.
