Comments:
An unannounced complaint investigation regarding staff qualifications, physical plant, staff to children ratios, diapering and behavioral guidance was initiated on January 13, 2020 at 2:00 p.m. to 3:30 p.m. Observation was conducted. Staff interviews were conducted. Center documents and staff records were reviewed. An exit interview was conducted on January 31, 2020 with the director.

Based on the information collected during the investigation, the complaint regarding staff qualifications, physical plant, diapering and behavioral guidance was determined to be invalid. The complaint regarding staff to children ratios was determined to be valid.

Description: Based on interviews, it was determined that the center did not follow the required one staff to four children ratio for the infant room at all times. Evidence: On at least one occasion each in October 2019, November 2019 and December 2019, there was one staff member with five infants for approximately 15 minutes at or around 5 p.m.

Plan of Correction: Staff are scheduled to meet the infant ratio and classrooms should never be out of compliance. This will not happen again
