Comments:

The licensing inspector conducted an unannounced monitoring inspection on 2/11/2020 from 10:45am to 4:20pm. The children were observed playing in the gymnasium, participating in Spanish class, eating lunch and preparing for nap. A formal observation was completed with the Flamingo's and Lion's classrooms. The lunch menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today, as well as the center's buses. Ten children?s records, ten employee records and five employee background check records were reviewed during this inspection. Medication is being administered and was reviewed. Ten injury reports were reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 1/17/2020 Last shelter-in-place drill: 09/12/2019 Last fire inspection: 08/01/2019 Last health inspection: 10/22/2019 The child to staff ratios were observed and all classrooms were in ratio. The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection. Standard #:	22VAC40-185-60-A
Description:	Based on record review, the center did not ensure one of ten children's records had documentation of child updates and confirmation of up to date information. Evidence: The record of child #10 (start date: 11/5/2018) did not have documentation of updates and confirmation of up to date information.
Plan of Correction:	We will have parent review and update information by email today. We will complete update by Wednesday, February 12th.
Standard #:	22VAC40-185-70-A
Description:	Based on record review, the center did not ensure two of ten staff members had documentation of all required information. Evidence: 1. The record of staff #5 (start date: 8/26/19) did not have documentation that the staff possesses the education required by the job position. 2. The record of staff #6 (start date: 11/4/19) did not have documentation that the staff possesses the education required by the job position.
Plan of Correction:	One of said staff members was corrected on site - inspector received proof of education. For other staff member, we will correct by Wednesday, February 12th.
Standard #:	22VAC40-185-500-B
Description:	Based on observation, the center did not ensure that diapers were disposed in a leak proof or plastic lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: The elephants classroom had soiled diapers in a trash can that was not foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

Standard #:	22VAC40-185-570-A
Description:	Based on observation, the center did not ensure that the protective belts were fastened for children placed in an infant seat or high chair. Evidence: During lunch time, the inspector observed three infants sitting in low feeding chairs without their protective belts fastened. This presented a fall hazard if a child were to climb out of the chair.

Description:	Based on record review, the center did not ensure one of fifteen staff record's contained documentation of an out of state child abuse and neglect search request by the end of the 30th day of employment. Evidence: The record of staff #3 (start date: 8/30/19) did not have documentation of an out of state child abuse and neglect search request by the end of the 30th day of employment, for one of the two states listed in the staff's record.
Plan of Correction:	We sent email to out of state central registry today. Will update employee file as soon as they reply.
