An unannounced monitoring inspection was conducted between the hours of 10:00 am and 12:30 pm. A sample of 3 staff and 5 children's records were reviewed, in addition to 3 medications. Children were observed during lunch and preparing for nap. Infants were observed sleeping in cribs, on the floor with staff and eating.

Evidence:
Staff #1 (hired 10/11/19) did not have documentation of an emergency contact.Plan of Correction: The following corrective action was provided by the director during the exit interview:

Standard #: 22VAC40-185-500-B Description: Based on observation the center did not ensure all requirements were met when diapering.
Evidence:
A staff in the infant room was observed changing a diaper and did not wash her hands afterwards.Plan of Correction: The following corrective action was provided by the director.
Evidence: The director spoke to the staff during the inspection. Hand washing will be reviewed with all staff.
