Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted on February 5, 2020. The on-site inspection was from 9:30 a.m. to 1:10 p.m. On this date, there were 40 children present in five classrooms and in the care of eight staff members. There was one additional staff member available to assist if needed. There were 83 children enrolled in the center. There were five staff members' records and five children's records reviewed. There were five classrooms toured and observations were made in two of those rooms. There were eight staff members and five children interviewed. The medication and medication paperwork were reviewed for one child. The exceptions to compliance were noted on the violation notice and the risk ratings for those violations were noted on the supplemental page. Lunch was observed to meet USDA guidelines. Some of items reviewed and observed during the visit included: choice time, Valentine art activities, story time, learning about teeth, a letter hunt, bathroom and hand washing procedures, infant care, diapering procedures, nap time, annual health and fire inspections, required posted information, documentation of evacuation, shelter-in-place, and lock-down drills, warm water temperature, first aid kit, and emergency supplies.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of five children's records and confirmation with the director, the center failed to obtain one child's immunization record before the child's first day of attendance.

Evidence:
1. The record for Child #4 contained an immunization record showing the child was adequately immunized which was dated 09/14/2018. The child's first date of attendance was 09/10/2018.
2. The director verified that the immunization record had been received after the child's first date of attendance.

Plan of Correction: The director will ensure that immunization records are obtained for all children before the child actually starts attending the center.

Standard #: 22VAC40-185-160-C
Description: Based on review of five staff records and verification from the director, one staff member did not obtain an updated tuberculosis (TB) screening within two years of the most recent screening.

Evidence:
1. The record of Staff Member #1 showed that the most recent TB screening was obtained on 01/14/2020. The date of the previous TB screening was 11/15/2017.
2. The director confirmed that the TB screening was not obtained within two years of the most screening.

Plan of Correction: The director will ensure that from now on, she will have staff obtain TB screenings within two years of the most recent screening.
