Comments:
Thank you for your time and assistance with the unannounced renewal inspection conducted on February 10, 2020. The on-site visit was held from 10:05 a.m. to 12:55 p.m. On this date, there were 38 children present in two Montessori classrooms and in the care of four to five staff members. One staff member was available to assist if needed. The total enrollment for the school was 40 children. There were five staff members' records reviewed and five children's records reviewed. Four staff members and four children were interviewed. Both classrooms were observed. The medication and medication paperwork were reviewed for two children. The exceptions to compliance were noted on the violation notice and the risk ratings for each violation were noted on the supplemental page. All food was provided by the parents. Some of the items reviewed and observed included: circle time, Montessori work activities, outdoor play, musical activities, story time, Valentine art activities, a heart lacing activity, hand washing and bathroom procedures, lunch, nap time, dismissal procedures, first aid kit and emergency supplies, documentation of evacuation and shelter-in-place drills, required posted information, injury reports, and warm water temperature.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on the review of five staff records and discussion with the director, three staff members failed to obtain an updated tuberculosis (TB) tests/screenings within two years of the most recent tests/screenings.

Evidence:
1. The record of Staff #2 contained an updated TB test dated 09/19/2019. The date of the previous test was 08/21/2017.
2. The record of Staff #4 contained an updated TB test dated 09/25/2019. The date of the previous test was 08/09/2017.
3. The record of Staff #5 contained an updated TB test dated 10/01/2019. The date of the previous test was 08/23/2017.
4. The director confirmed the dates between the two TB tests for all three staff members were over two years.

Plan of Correction: The director will ensure that all staff obtain follow-up TB tests or screenings within two years from their current one.

Standard #: 22VAC40-185-500-A
Description: Based on observation and discussion with staff, one staff member failed to wash her hands and the hands of a child after the child's diaper was changed.

Evidence:
1. While wearing disposable gloves, Staff A finished changing a child's diaper and removed the gloves, but she did not wash her own hands or the hands of the child.
2. Staff A acknowledged that she did not wash her hands or the child's hands after diapering.

Plan of Correction: The director will retrain all of the staff on proper diaper changing techniques and will post the proper techniques in the diaper changing area.

Standard #: 22VAC40-185-500-B
Description: Based on observation and verification from staff, one staff member failed to use the foot-operated diaper pail and failed to clean and sanitize the diaper changing pad after diapering a child.

Evidence:
1. After diapering a child, Staff A used her hand to open the diaper pail and not the foot pedal when throwing away the dirty diaper.
2. Staff A did not clean the diaper changing pad with soap and water and did not sanitize the pad after changing a child's diaper.
3. Staff A acknowledged that she did not use the foot pedal on the diaper pain and did not clean and sanitize the changing pad.

Plan of Correction: The director will retrain all of the staff on proper diaper changing techniques and will post the proper techniques in the diaper changing area.
