Technical Assistance:
The following were discussed with the program director:
- storage and retention of medication administration records
- offering of second helpings of food to toddlers
- serving of meals to toddlers (staff sitting with the children)

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 10:50 AM and departed at 1:15 PM. Twenty children were in care. Morning activities, lunch service and nap time were observed. A sample size of five staff records and five children's records were reviewed. Medication administration and injury records were reviewed.
An exit meeting was conducted with the program director prior to closure of the inspection.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based upon review of five children's records and staff interview, the facility has not ensured that each child's record includes documentation of a physical examination by a physician before attending or within 30 days after attending.
Evidence:
1. The record provided for child 1, who, according to the record, began attending on 09/15/2019, did not include documentation of a physical examination.
2. Staff 1 verified that the record for child 1 did not include documentation of a physical examination.

Plan of Correction: The facility responded with the following:
All children's records will be checked to ensure that documentation of physical examinations are in the records. The parent of child 1 was contacted during the inspection.

Standard #: 22VAC40-185-270-A
Description: Based upon observation, the facility has not ensured that areas are maintained to be safe.
Evidence:
There are several areas along the back wall of the building along the playground where the stucco/paint is chipping/peeling off of the building. These areas are within reach of the children.

Plan of Correction: The facility responded with the following:
The chipping areas will be sanded and new cement will be added over the areas.

Standard #: 22VAC40-185-550-M
Description: Based upon review of the injury records, the facility has not ensured that all required documentation is included on the record.
Evidence:
An injury record dated 02/12/2020 for child 6 did not include the time or method by which the parent was notified.
