An unannounced monitoring inspection was completed on 02/18/2020. There were 23 children, ages 4 years-11 years, and 5 staff members present during the inspection. The inspector reviewed 5 children's records, 5 staff records and 3 medications during the inspection. The children were observed in the following activities: arriving to the program, in large motor play in the gym, having an afternoon snack, participating in an art project and during homework time. The inspector reviewed the Injury Prevention Plan, Playground Safety Plan and the emergency supplies during the inspection. The inspector discussed nutrition and sharing space with the school with the staff. Based on record review and discussion with the staff, the facility failed to ensure that staff records contain all of the elements as required by the standards,
Evidence: The record for Staff #5 did not contain the job title for Staff #5.Plan of Correction: The program director will get the job title from the program administrator.
Standard #: 22VAC40-185-500-A Description: Based on observation and discussion with the staff, the facility failed to ensure that children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.
Evidence: The children in Group 2 did not wash their hands with soap and running water or a disposable wipe after eating their afternoon snack. Students began to do their homework or participate in an art project immediately following eating.Plan of Correction: Remind the staff to wash hands after snack.
