The inspector conducted an unannounced renewal inspection from 10:00 a.m. until 11:30 p.. at the center and then from 1:00 p.m. until 2:00 p.m. at the main office. Five children's records were reviewed and four staff records were reviewed. Children were not in care due to the school announcing the previous night all county schools would be closed the rest of the week due to illness. Violations are identified on the violation notice. Standard #: 22VAC40-185-270-A Description: Based on observation, thee center failed to maintain the center in a safe condition.
Evidence:
1. The inspector observed a broken wheel and missing pedals on the red piece of play equipment on the playground.Plan of Correction: It will be removed; it is not currently used by children.
Standard #: 22VAC40-185-330-B Description: Based on observation and measurement, the center did not meet the minimum safety standards for fall zones on the playground.
Evidence:
1. The inspector measured the fall zone for the gray climbing equipment and there was 5 feet of mulch; the requirement is 6 feet on all sides.Plan of Correction: The center is planning on getting new playground equipment and mulch.
Standard #: 22VAC40-191-40-D-1-A Description: Based on interview, the center did not obtain central registry results by the end of the 30th day of service for an agent.
Evidence:
1. The inspector was told that the central registry check has not been completed.Plan of Correction: The agent has been given the info. and has been reminded it needs to be completed.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on interview, the center did not have have criminal history records prior to the first day of employment/service for a board agent.
1. The inspector reviewed one background check for an agent and was told the fingerprinting had not been completed.Plan of Correction: The board agent has been given the paperwork to complete and has been reminded to complete it.
