63.2(17) License & Registration Procedures

Comments:
An unannounced renewal inspection was conducted from 11:00 am- 5:45pm. At the time of entrance, 101 children were in care with 16 staff members present. Children were observed during music time, engaged in free play, and learning Spanish words with flash cards. Transitions, diapering and handwashing was also observed. A selection of 10 staff and 10 children records, the physical space, evacuation drills, attendance records, injury reports, medications, the Injury Prevention Plan, and the Emergency Preparedness Plan were reviewed. Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for one child under the age of two years.

Evidence:
1. Child #3 is under the age of two. The most recent documentation of immunizations in the record for Child #3 was dated 7/31/19.

Plan of Correction: The parent was contacted during inspection.

Standard #: 22VAC40-185-160-C
Description: Based on record review, the center did not obtain documentation of a follow-up tuberculosis screening every 2 years for two staff members.

Evidence:
1. The tuberculosis screening documented in the record for Staff #1 was dated 1/16/18.
2. The tuberculosis screening documented in the record for Staff #6 was dated 2/9/18.

Plan of Correction: Staff #1 is no longer employed at the center. Staff #6 was informed during inspection and will get a new tb screening.

Standard #: 22VAC40-185-240-A
Description: Based on record review, 3 of 10 staff records did not contain documentation of recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by the Code of Virginia by the first day of employment.

Evidence:
1. There was no documentation of child abuse and neglect training in the records for Staff #1(start date 7/12/19) and Staff #2 (start date 10/30/19).
2. The certificate for child abuse and neglect training in the record for Staff #3 (start date 7/24/19) was dated 8/30/19.

Plan of Correction: Staff #1 is no longer employed by the center. Staff #2 will complete the training by close of business tomorrow.

Standard #: 22VAC40-185-550-M
Description: Based on record review, 2 of 7 injury reports did not contain all required information.

Evidence:
1. An injury report for Child #1 and Child #2 did not contain future action to prevent recurrence of the injury.

Plan of Correction: We will put a reminder in the staff newsletter tomorrow about making sure staff fill out the section for future actions to prevent injuries and our business manager will review all injury reports before they go in the files.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the center did not have a central registry finding within 30 days of employment for two staff.

Evidence:
1. Staff #4 began employment on 7/12/19. The central registry finding for Staff #4 was dated 12/17/19. Documentation in the record indicates the central registry paperwork was mailed out on 11/18/19.
2. Staff #5 began employment on 8/19/19. The central registry finding for Staff #5 was dated 12/17/19. Documentation in the record indicates the central registry paperwork was mailed out on 11/18/19.

Plan of Correction: The center mailed central registry paperwork on 11/18/19 upon realizing they had not been sent. Procedures for obtaining central registry findings were changed.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on record review, an out of state child abuse and neglect search was not completed.

Evidence:
1. Agent #1 resides in Maryland.
2. No Out of State background search results were obtained from the state of Maryland.

Plan of Correction: Agent #1 was contacted during inspection. The center will provide documentation of Out of State Child Abuse and Neglect findings to the Department once received.
