Technical Assistance:

Comments:
An unannounced monitoring inspection was conducted on 2/19/20 from 2:15pm - 4:10pm. During the inspection there were 44 children ages four years old to ten years old in care with three staff. Children were observed participating in various activities in the cafeteria and the gym, eating dinner, playing outside and doing homework. Records were reviewed for five children while at the facility. The records for four staff were reviewed on 2/20/20 at the central office. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of five children's records, it was determined that the facility did not ensure that the center obtains documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
1. There was no record for child #5, present during the inspection, so there was no immunization record available for viewing during the inspection.
2. Staff #1 (Program Director) confirmed there was no immunization record available for viewing during the inspection.

Plan of Correction: The facility responded: The facility responded: When child #1 was registered for the program, the wrong location was entered and his record was sent to that location. We will bring his record, which contains the immunization record, to this location.

Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information.

Evidence
1. The was no record for child #5 (date of enrollment 2/18/20), present during the inspection, available for viewing during the inspection.
2. Staff #1 (Program Director) confirmed that the record for child #1 was not available for viewing during the inspection.

Plan of Correction: The facility responded: When child #1 was registered for the program, the wrong location was entered and his record was sent to that location. We will bring his record to this location.
