Comments:

A Subsidy Health and Safety inspection was conducted on 2/19/20 from 2:15pm - 4:10pm. During the inspection there were 44 children ages four years old to ten years old in care with three staff. Children were observed participating in various activities in the cafeteria and the gym, eating dinner, playing outside and doing homework. Records were reviewed for five children while at the facility. The records for four staff were reviewed on 2/20/20 at the central office. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description:	Based on a review of four staff records, it was determined that the vendor did not ensure that within the first 90 days of employment all staff shall complete the Virginia Preservice Training for Child Care Staff. Evidence: 1. The record for staff #4 (date of hire 11/14/19) did not include documentation to demonstrate complete of the all staff shall complete the Virginia Preservice Training for Child Care Staff. 2. Staff #5 reviewed the record for staff #4, and confirmed that staff #4 did not complete the Virginia Preservice Training for Child Care Staff.
Plan of Correction:	The facility responded: Staff #4 will be required to complete the Virginia Preservice Training for Child Care Staff. All new staff will complete the Virginia Preservice Training for Child Care Staff within 90 days of employment.

Description:	Based a review of four staff records, it was determined that the vendor did not ensure that all staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Evidence: 1. The record for staff #4 (date of hire 11/14/19) did not contain documentation for a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. 2. Staff #5 reviewed the record for staff #4 and confirmed that staff #4 did not have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.
