Discussed: ensuring that tables are allowed to air dry for at least one minute after sanitizing solution is sprayed on and using paper towels to dry the excess wetness from the tables after they have air dried.

An unannounced monitoring inspection was conducted from 4:00 - 5:30 pm with staff. There were 18 children, ranging from five years to 11 years of age, present with two staff supervising. Children were observed eating snack, washing hands, reading and participating in a group literacy activity. Snack served today: salad with cheese and ranch dressing and water. Five children's records were reviewed. Two on-site staff records were reviewed and in compliance. Staff stated that there was no medication on site. Staff have current certification in Medication Administration Training (MAT), first aid/CPR and Daily Health Observation (DHO) training. The emergency drill log was reviewed and in compliance. Standard #: 22VAC40-185-140-A Description: Based on a review of five children's records, the facility failed to obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance. Evidence: the records for child #1 (date of first attendance: 8/12/19) and child #5 (date of first attendance: 8/12/19) did not have documentation of a physical examination. Plan of Correction: Will ask parents to provide documentation of a physical.
