Comments:
An unannounced monitoring inspection was conducted today between 10:00 am and 2:30 pm. There were 77 children and 14 staff members. The children were observed working on table toys and games. Children were observed hand-washing and preparing to go outside to the playground. The physical plant, children's records, staff records, evacuation plans, cribs, bottles, evacuation logs, attendance records,emergency supplies and medications were inspected. Standard #: 22VAC40-185-60-A
Description: Based on a review of child records, the center failed to obtain and maintain required documentation for each child enrolled.

Evidence:
Child #1 and #4's records did not include documentation of work phone number and place of employment of each parent.

Plan of Correction: I will get information from parents and place in children's records.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records, the center failed to obtain and maintain
all required records.

Evidence:
Staff #2 and 3's records did not contain documentation of emergency contact information.

Plan of Correction: I will collect information from staff and place in employee records.

Standard #: 22VAC40-185-240-A
Description: Based on a review of staff records, the center failed to ensure that records contained documentation of orientation to verify training received by the end of the first day of assuming job responsibilities.

Evidence:
Staff #2, #3 and #3's records do not include documentation of orientation to verify training received by the end of the first day of assuming job responsibilities.

Plan of Correction: The Center Director states that she knows that staff completed orientations and that she will continue to look for the documentation and will contact the licensing inspector once she has located the paperwork.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint.

Evidence:
In the Jr. Toddler classroom, the paint is peeling off of the changing table.

Plan of Correction: We are going to replace the changing table.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medications, the center failed to ensure adequate methods to prevent the usage of outdated medications.

Evidence:
1. A medication had expired November 2019. 2. The Center Director verified that this medication was still on site.

Plan of Correction: The Center Director stated that this medication will be removed from the site by 2/26/2020.
